Pick the plan, payment option, and delivery site below and then mail in the form (or just hand write your selction of options on a scrap peice of paper) along with, full payment or registration deposit. Or if you prefer, e-mail allie@ArugulasStarFarm.com the required information below and then send your full payment or registration deposit by mail. If you choose not to pay in full, please remember to note on your calendar when the other dates of payments are due so we do not have to remind you.

½ Bushel Plan payment option:

#1)____Full payment of $810.00 + $11.50 for

baskets due upon registration

#2)____ Payment plan: $210.00

deposit + 11.50 for baskets due

upon registration. Then $300.00 due on

April 29th & $300.00 due on May 13th^.

#3)____Monthly Plan: $176.95 + $11.50 for

baskets due upon registration. Then

$92.57 due at the first of each month’s

delivery.

Bushel plan payment option:

#1)___Full Payment of $1,620 + $13.50 for

baskets due upon registration

#2)___Payment plan: $620.00 +$13.50

for baskets due upon registration. Then

$500.00 due on April 29^th, &

$500.00 due on May 13^th.

#3)___Monthly Plan: $276.95 + 13.50 for

baskets due upon registration. Then

$194.00 due at the first of each month’s

delivery.
